Manx Care launches Intermediate Care Service

Aims to reduce unnecessary hospital admissions

A new initiative to prevent unnecessary acute hospital admissions and support timely discharges from hospital has come into effect.

Manx Care says its Intermediate Care Service will result in a reduction in both the number of patients presenting in the Emergency Department, and those occupying hospital beds who don't need to be there.

Health officials say the redesign of current services will bridge the gap between community and secondary care provision. 

It's a multi-disciplinary approach providing short-term rehabilitation for people who've experienced a decline in their abilities for various reasons, helping them to return to their maximum possible level of independence.

The model will see Advanced Clinical Practitioners working within the Emergency Department, hospital wards and the community to ensure that time critical assessments can be completed via a single point of referral.
