Patients reminded to go prepared
People going into hospital are being reminded to ensure they take any medications they use in with them.
This includes prescribed and purchased medicines, such as over the counter remedies and herbal products.
Manx Care says it's important the pharmacy team can understand what patients are already taking.
It says having your own medicines will:
- reduce medicine-related errors by increasing the quality of your medicine history notes
- ensure medicines prescribed on admission correspond to those you were taking before admission
- reduce hospital discharge delays
- reduce omitted and delayed doses
- maintain the familiarity of your own medicines
- move medicines along the care pathway with you
- reduce the storage of too many medicines at home
- help healthcare staff understand how patients manage their medicines at home
The main aim of this is to keep all medicine, including those from home, in one place.
