The Team Sky rider won a bunch sprint into the Adriatic resort of Fano.
After opening up with 150 metres to go to the line, he held off a late challenge from former HTC team-mate Matthew Goss to win by a bike length.
Cav has won two stages at this year's Giro taking his career total to nine - it's his 80th win since becoming a professional rider in 2007.
