Almost £140,000 is owed by non-Island residents who've used hospital services while visiting the Isle of Man.
Since 2017, invoices totalling almost £600,000 have been issued to visitors who've received healthcare while on the Island.
In a Tynwald written answer, Health Minister Claire Christian confirmed £139,229 remains outstanding.
The figures for 2024-25 show more than £99,000 unpaid, the majority of which relates to one case which is being pursued through the courts.
A reciprocal health agreement between the United Kingdom and Isle of Man means the majority of hospital services are non-chargeable for UK visitors.
