Chief officer of 'Teal Bay' died in August last year
A report has been issued by the UK's Marine Accident Investigation Branch into a fatality aboard an Isle of Man registered ship in August last year.
The chief officer aboard the bulk carrier Teal Bay died when he was struck by a mooring line.
It happened at the KavKaz South Anchorage in Russia.
The MAIB report says the officer was standing in a hazardous area close to a tensioned mooring line during an operation to move the vessel forward.
This, the report says, was attempted with insufficient crew and without risk assessment.
Furthermore a lack of a coordinated, and organised, emergency response created delays in the chief officer being assessed by a medical professional.
The Marine Accident Investigation Branch produced the report on behalf of the Isle of Man Ship Registry.
